What is periodontitis?

Periodontitis is an inflammatory disease of the tissues supporting the teeth (gum, periodontal ligament, cementum, alveolar bone). It is caused by bacteria in tartar and plaque below the gum line. Without treatment it can lead to tooth loss even in otherwise healthy patients.

Stages – from gingivitis to periodontitis

  • Gingivitis – early stage. Gums are red, swollen and bleed when brushing. Fully reversible with correct treatment.
  • Mild periodontitis – beginning of bone loss. Pocket depths of 4–5 mm.
  • Moderate periodontitis – progressive bone loss, pockets 5–7 mm, teeth may begin to loosen.
  • Severe periodontitis – significant bone loss, pockets >7 mm, tooth mobility, risk of tooth loss.

Treatment methods

The treatment plan is determined by the stage of disease:

  • Scaling and root planing (SRP) – deep ultrasonic cleaning below the gum line. The primary non-surgical treatment for periodontitis.
  • Curettage – mechanical cleaning of pockets to remove infected cementum and granulation tissue.
  • Periodontal surgery – for advanced periodontitis. Includes flap surgery and guided tissue regeneration.
  • Supportive periodontal therapy – cleaning every 3–4 months to maintain results after active treatment.

Is the treatment painful?

Deep cleaning is performed under local anaesthesia and is painless. Some mild sensitivity for 2–3 days after is normal. Periodontal surgery is also performed under anaesthesia.

How many visits are needed?

Active treatment usually involves 2–4 visits for deep cleaning, followed by a reassessment at 6–8 weeks. Advanced cases may require surgery.

Can I lose teeth from periodontitis?

Yes – untreated periodontitis is the leading cause of tooth loss in adults. This is why early treatment and regular supportive therapy every 3–4 months are so important.
